As people age, the effects of gravity, exposure to the sun, and the stresses of every day life can be seen in their faces. Facial rejuvenation encompasses a wide variety of procedures, which includes face lifting, brow lifting, mid face lifting, implants for facial enhancement, and laser rejuvenation. We are now utilizing many minimally invasive techniques for facial rejuvenation including fat transfer for facial augmentation, endoscopic brow and mid face lifting, and laser and plasma resurfacing.  These procedures are customized to fit each individual’s condition, skin type, and expectations.

 

Facelift

A facelift (technically known as rhytidectomy) can improve the most visible signs of aging by removing excess fat, tightening underlying muscles, and redraping the skin of your face and neck. A facelift can be done alone, or in conjunction with other procedures such as a forehead lift, eyelid surgery, laser treatments for fine wrinkles, and or nose reshaping.  The best candidate for a facelift is a man or woman whose face and neck have begun to sag, but whose skin still has some elasticity and whose bone structure is strong and well-defined. A facelift can make you look younger and fresher, and it may enhance your self- confidence in the process.

Brow Lift 

Browlift / Eyelift also known as Blepharoplasty or eyelid reshaping: Blepharoplasty involves the reshaping of tissues of the upper eyelid, and the removal or redistribution of fat from the lower eyelids. We specialize in the transconjunctival lower lid procedures, which leave no external scars. Combinations of procedures are utilized with blepharoplasty surgery, including laser resurfacing of the lower eyelid skin, and in many cases, endoscopic browlifts. Many blepharoplasty procedures are performed in combination with other facial rejuvenation procedures such as browlifts and laser resurfacing.

Erbium Laser Resurfacing

In laser skin resurfacing a laser is used to remove areas of damaged or wrinkled skin, layer by layer. The procedure is most commonly used to minimize the appearance of fine lines, especially around the mouth and the eyes, and is especially effective for treating acne and facial scars or areas of uneven pigmentation. Laser resurfacing may be performed on the whole face or in specific regions. Often, the procedure is done in conjunction with another cosmetic operation, such as a facelift or eyelid surgery. The technique directs short, concentrated pulsating beams of light at irregular skin. Laser skin resurfacing removes skin very precisely, layer by layer, resulting in fewer problems with hypopigmentation (lightening of skin). This popular procedure is known by several other names, including laserabrasion, laser peel or laser vaporization.

Who Is a Good Candidate For Laser Resurfacing?

If you have fine lines or wrinkles around or under the eyes, the forehead, the mouth, or scars from acne, non-responsive skin after a facelift, then you may be an ideal candidate for laser skin resurfacing. If you have active acne or if you have very dark skin, you are not an ideal candidate.

Portrait® Plasma Resurfacing

Portrait® plasma resurfacing is not a laser, pulsed light, or radio frequency procedure. Fat Grafting also known as Liposculpting of the Face

Fat grafting is the procedure where your own fat is utilized to augment, rejuvenate or correct deficiencies in the face that occur with aging.  It results in a more natural rejuvenation and “refreshed look” since there is no skin tightening--only the replacement of natural tissue that is lost in the aging process. It is now routine to use your own body fat to augment deep lines, improve weak chins and cheeks, and increase the volume and shape of the lips.

The first of these developments is the use of a centrifuge to prepare the harvested fat before re-injection. This is performed while the patient is under twilight anesthesia. The second development is the addition of autologous platelet gels to fat grafting. This involves harvesting blood before the procedure, removing the platelets with a high-tech centrifuge, and mixing this with the purified fat prior to re-injection.
The addition of platelet gels has a number of wonderful benefits. By using the platelets from your own blood and integrating them into the fat grafts, there is a significant reduction in bruising and swelling, improvement in the integration of the harvested fat cells into the face. Both of these techniques are described in detail in two enclosed articles.

Liposculpting involves removing fat from specific areas of the face using very fine cannulas. This leaves minimal bruising, swelling and disfigurement, and results in sculpted areas looking more desirable and less full of tissue, especially in areas of excess fat.

The rate of re-operation is approximately 15%, primarily due to the patient's desire for further augmentation.
There are very few risks. These include bruising, swelling, asymmetry, and, very rarely, infection, injury to nerves resulting in either weakness of facial muscles or numbness.

Most procedures are performed under local anesthesia with intravenous sedation. If other procedures are performed at the same time, general anesthesia may be used. We will provide all of the appropriate medications in the form of prescriptions for your procedure.

After the surgery, the patient can expect 5 to 7 days of swelling and bruising but very little pain or discomfort. The fat is typically harvested from the area around the abdomen or hips. Dressings may be removed on the second postoperative day. Foam tape is used to stabilize the grafts in the facial regions. We will keep these on for 2 to 3 days.

Scar Revision

In addition to cosmetic surgery, we specialize in primary and secondary reconstruction of facial defects that are caused by trauma or diseases. This can include skin cancer removal and reconstruction, treatment of facial bone fractures, secondary reconstruction of nasal defects either from trauma or prior surgery, revision of scars from accidents or prior surgery or from diseases that cause facial defects

Ear Pinning

Although otoplasty, ear pinning surgery, is typically performed on younger children (ages 4-14 or so), it is frequently done for adults as well. It is a procedure that will bring the ear closer to the head and reduce the size of seemingly large ears.  It is also used to correct “cup” ear deformities in which the ears develop without their natural folds.
The procedure itself is fairly simple. An incision is made in the back of the ear so the cartilage can be sculpted and folded, allowing the ear to lie closer to the head. Stitches are used to close the incision. The head will be wrapped with a rather bulky bandage for several days. This will help the newly sculpted ear to mold and heal properly. After the healing process, the result will be a more natural look to the head overall.
